CIBC forms: where to send

After filling out any form from the bank, you can send it in one of the following ways:

1. by e-mail:

• call at 1-800-567-3343
• CIBC will send you a secure e-mail
• send the form to the received e-mail

2. bring the completed form to one of the CIBC branches
3. mail the filled form to CIBC Investor Services Inc, 161 Bay Street, 4th Floor, Toronto, Ontario
